WORKING WITH CASE MANAGERS


Case Managers and Physiotherapists share a common goal of supporting patients in achieving their best recovery after a life changing injury. This blog will delve into how we align treatment strategies and work in collaboration with Case Managers to provide the highest level of care.

UNDERSTANDING THE EVIDENCE


The traumatic injury NICE guidelines include how to treat spinal, nerve, musculoskeletal, visceral, soft tissue, and limb injuries. The rehabilitation needs should be assessed at an early stage and early invention is key. The guidelines recommend a multi-disciplinary team assessment, this is frequently completed as an inpatient or in a rehabilitation centre, however, this should not stop when a patient returns home. We value building therapeutic relationships with other professionals. The ability to meet virtually, provides a cost effective way of bringing together large multi-disciplinary teams when the patient is receiving care in the community. Our therapists are set up with equipment to be able to complete meetings with a professional setup.
We are part of a collaboration called MDT Essex, which is a cooperative of companies.MDT Essex Home is a group of experienced disability focused companies who can provide equipment, Occupational therapy, and Physiotherapy.

GOAL SETTING


The NICE guidelines recommend that having clear goals to strive for can help stop patients feeling overwhelmed. It also helps direct the multidisciplinary team towards a common aim and has been proven to improve outcomes for patients following major trauma.
At Estuary Physio, our goals are led by the long-term goals set out by the client and Case Manager in the initial referral, we will work with the patient to break these down into short-term Physiotherapy goals and actions. We will communication with the Case Manager on progress and supporting outcome measures over phone call, email or in our progress reports. If we feel that we are unable to achieve goals or the patient is not making progress, we will collaborate with the team to refine the treatment approach. Communication is key to providing coordinated care, we take a proactive response to the inevitable hurdles that occur through any rehabilitation journey.

SPECIALIST REHABILIATION


We understand that our patient often have a range of injuries. Our therapists have a broad range of backgrounds including the NHS, charity, and private sector. We can provide curriculum vitaes on request. We do not have inhouse Physiotherapy assistants, however, we are able to provide double handed care to patients with a company we partner with. Barry Ford, Physiotherapist/ director has experience working with case managers in inpatients and the community, he therefore understands the litigation process and the role a Physiotherapist plays within this area. He oversees all cases and reports.
As a team, we are used to bringing our different experiences together to provide the best outcomes. If for example, a patient needs a specialist Neurological Physiotherapist and Musculoskeletal Physiotherapist, we will try to keep these under the Estuary Physio team to aid with communication through notes. If we are unable to provide a treatment, we will support the Case Manager with finding the right person for the job.

FLEXIBILITY


We understand that the road can be a long one for Case Management managed patients, we are happy to organise a virtual discovery call, free of charge, where patients can chat with us to find out more about our service and if we are the right match for them. We treat patients in a range of settings, including at hospital, home, the gym, or work. As the patient progresses through their journey, we can meet the needs of our patients in the place and at a time that suits them. We can pause private rehabilitation if someone requires an inpatient stay or is well supported through NHS Physiotherapy services.

COSTS


We clearly lay out our costs and estimate the number of sessions of therapy are needed to achieve the agreed goals in the initial report and progress reports. If we anticipate any changes in costs, we will speak with the Case Manager, directly, prior to completing work. We provide timely invoicing, with a breakdown of session dates. In house, we keep a track of the number of sessions allocated to ensure we do not go over and reports are sent out when due.
